  2. Complete Intake and Consent Forms
    If you decide to move forward, you’ll receive access to our secure online portal to complete:
    • Practice rules and regulations
    • HIPAA information
    • Consent to treat
    • Payment method information
  3. Schedule Your Initial Consultation
    Once your forms are complete, you’ll schedule a 60-minute initial session to begin your personalized care plan.

    Note: Mental health services may be provided virtually or in person depending on your needs.
